Heart Failure Quality Standard
Overview
Integrated heart failure care is not only about the model of care delivery, but also the quality of clinical care delivered. The Integrating Heart Failure Care Initiative (IHFCI) involved the implementation of both the Spoke-Hub-Node model of heart failure (HF) care, as well as a standard of HF care, as guided by Health Quality Ontario’s Quality Standard for Heart Failure in the Community for Adults. Below are several tools and resources to help you and your team understand the HF Quality Standard, and bring the 10 quality statements into action, through your care integration endeavours.

HQO’s Heart Failure Care in the Community for Adults Quality Standard
The Heart Failure Care in the Community for Adults Quality Standard was released by Health Quality Ontario in February 2019. This quality standard includes 10 quality statements which address care for adults who have HF. It applies to community settings, including primary care, specialist care, home care, hospital outpatient clinics, and long-term care.
HQO’s Recommendations for Adoption
The Recommendations for Adoption is a document aimed at supporting local and system-wide level uptake of the Quality Standard. Developed through extensive review and consultation with experts, these recommendations highlight what can be done to help bridge the gap between practice and the Quality Standard.
HQO’s Getting Started Guide to Implement Quality Standards
The Getting Started Guide is a comprehensive process to help clinicians, administrators, and any others in the health care system, including patients and caregivers, use the Quality Standard.
