Change Package (working document)

The Change Package accompanies the Ontario Framework for Lower-Limb Preservation and is a working document. It provides actionable change ideas, tools and resources from which lower-limb preservation providers, organizations and champions can select to improve lower-limb preservation care delivery, services, and outcomes. The change ideas, tools and resources can be adopted and/or adapted and tested regionally to determine whether they result in improvements. The Change Package will be expanded and co-developed by CorHealth and lower-limb preservation stakeholders to support provincial scale-up.
