Ontario Vascular Program Evaluation Tool
In 2016, CorHealth Ontario worked with hospitals to provide Level 1, 2, or 3 designations to acute care vascular programs across the province using the ‘Ontario Current State Assessment and Proposed Program Framework: Acute Care Vascular Services’ as the guide. Since then, CorHealth has continued to focus on opportunities to advance vascular care through driving evidence-informed practice; and, informing planning, access, and resource allocation. Through ongoing engagement with vascular stakeholders, we identified the need to refresh the designated vascular program levels to determine current status of capacity, coverage, expertise, and resources.
In response, CorHealth has updated the Ontario Vascular Program Evaluation Tool (Evaluation Tool) to ensure it is streamlined and easy to use while also reflects the essential information required to confirm designation and differentiation of Level 1, 2, and 3 Vascular Programs. With a focus on quality, coordination of care, and patient outcomes, the Evaluation Tool outlines the core service criteria required for each of these levels, and seeks to confirm fulfillment of the criteria by each of the vascular programs in the province.
CorHealth Ontario will be working with vascular hospitals over the coming weeks to complete the Evaluation Tool and confirm each hospital’s vascular program designation.
Following assessment of submitted Evaluation Tools, CorHealth will share the vascular program designation with the Vascular Leadership Council, each hospital and the Ministry of Health and Long-Term Care. In addition, information provided in the Evaluation Tool may be shared with other system partners such as CritiCall Ontario.
