Integrated Care Bundles

In the Fall of 2018, the Ministry of Health and Long-Term Care (MOHLTC) communicated their direction for bundled care funding policy in 2019/20, involving integrated care for selected hospital-based acute and post-acute services. Work is focusing on a Coronary Artery Bypass Graft (CABG) bundle, a stroke bundle, and an Aortic Valve (AVI) Implantation model. Implementation would begin with voluntary participation in 2019/20 and with the expectation of broader adoption in subsequent years. CorHealth Ontario (CorHealth) was asked to support the development of patient cohorts, evidence-based clinical pathways, and outcome measures for cardiac and stroke integrated care bundles.

Integrated care bundles are one tool the MOHLTC is employing to address the provincial bed crisis and “hallway medicine”, while remaining focused on the ‘Quadruple Aim’:

  • Improved patient experience
  • Improved cost of care
  • Improved population health
  • Improved provider experience

CorHealth is working closely with the MOHLTC, Health Quality Ontario (HQO), the Ontario Hospital Association (OHA), the Rehabilitative Care Alliance (RCA) and system stakeholders to support the provincial implementation. CorHealth will participate in the Communities of Practice led by HQO and provide coaching and support to stakeholders regarding bundle adoption and performance monitoring. Many hospitals have volunteered for implementation in 2019/20 and we look forward to working with them and providers across the continuum to gather and share tools and knowledge that support successful implementation and continued evaluation and refinement.
